The version and OS information reported by the apps used on each phone involved in the test
For issues related to Exposure Notifications:
(missing notifications, errors during code submission, etc.)The dates used for the “Testing Date” and “Symptoms Onset” fields when generating a Verification Code (or who provided the code to you if you did not generate it yourself)
The date and time when you submitted the verification code
The date(s) and time(s) describing how contact was simulated (when were the phones near each other)
The date(s) and times(s) the “Check for exposures” button was used
